Renewing a temporary or long-term placard

If you are renewing a red temporary parking placard or a blue long-term parking placard:

Step 1. Fill out an application

  1. Save the PDF form to your computer – click or right-click the link and download the form.
  2. Open the PDF form with Adobe Reader. Fill it in and save it.

Step 2. Get the application approved

The application may be approved by an authorized healthcare provider, such as a:

  • physician
  • occupational therapist
  • physiotherapist
  • surgeon
  • physical therapist
  • podiatrist
  • nurse practitioner
  • chiropractors

Step 3. Submit the application

Bring your completed application to a registry agent, along with:
