Sample 1: Request to Amend a Given Name

When Jo-anna was born, her parents completed a Registration of Birth recording their daughter's given name as Jo-anna. Over time (but within the first 10 years of Jo-anna's life) the spelling of her given name changed to Johanna.

To amend the birth record, Vital Statistics requires the following:

  • Proof to support the correct given name (Note: Supporting proof/evidence must show that Jo-anna used Johanna prior to her 10th birthday. Some examples of acceptable proof/evidence are a school record, an immunization card, baptismal/christening certificate.)

  • Complete a statutory declaration (prepared by Vital Statistics)

  • Fees (if applicable)

When all the requirements have been met, Vital Statistics finalizes the amendment to the Registration of Birth.

Note:  

  • If Jo-anna is not yet 10 years old, a statutory declaration will be prepared, supporting proof/evidence is not required  and a fee is applicable

  • If Jo-anna is 10 years old or older and cannot provide proof/evidence, showing she used the spelling of Johanna before her 10th birthday, she cannot proceed with the amendment (her only option is to proceed with the legal change of name procedure)

Sample 2: Request to Amend a Place of Birth

A bride's place of birth is incorrectly recorded on her Registration of Marriage as Edmonton, Alberta. Her correct birthplace should be recorded as Regina, Saskatchewan.

To amend the marriage record, Vital Statistics requires the following:

  • Proof to support the correct place of birth (some examples of acceptable proof/evidence are Saskatchewan birth certificate, passport, etc.)

  • A completed statutory declaration (prepared by Vital Statistics)

  • Fees (if applicable)

When all the requirements have been met, Vital Statistics finalizes the amendment to the Registration of Marriage.
